If you have already completed your questionnaire(s), thank you!
If you haven’t yet done so, it’s easy – and the questionnaire is short! Just go the tech renewal database and log in with your IC username and password, and select “View and Modify Assets Assigned to You.”
This will bring up a list of assets, with a “Questionnaire due” link in red for each asset scheduled to be replaced. Lab computers are 'grouped' according to room, so just one questionnaire needs to be completed per lab.
It is important that we have all of the questionnaires completed by the due date. Please note that beginning this summer, any MacPro system for which we do not receive a completed questionnaire will, by default, be replaced with an iMac.
If you are submitting a change request that generated a chargeback form, we also need to have the completed and approved forms submitted to us by that same date.
