Do you have dependents over the age of 19?
Our Dental, Accidental Death & Dismemberment, Life Insurance and Dependent Life insurance cover dependents over 19 (and under 25) as long as they are full time students or are registered disabled. Verification of full time student status is necessary to ensure eligibility requirements are met. If you have a dependent over 19 who is a full time student please let the Office of Human Resources know by completing a Dependent/Contact Update Form and enclosing documentation that verifies this information (a printed schedule, tuition bill etc.). If your dependent over the age of 19, does not meet eligibility requirements for the plans above, please contact Human Resources to update your benefits elections at benefits@ithaca.edu
If you have a dependent over the age of 19 who received Dependent Tuition Remission or a Cash Award for the 2011-2012 academic year, it is not necessary for you to complete the Dependent/Contact Update Form. Your application will be used as verification of full time student status.
Tuition Remission Applications should be submitted by June 1, 2011. Cash Award Applications should be submitted no later than July 15th and must be accompanied by a copy of the institutions bill showing a charge specifically for tuition. Cash Awards will not be processed without a tuition bill from the institution your dependent is attending. Our Dental Insurance provider, First Ameritas, may request full time student status verification as part of their claims processing procedure. It is your responsibility to provide this information to First Ameritas, if requested.
