Please dress appropriately to be able to receive a shot in the upper arm!
In order to greatly expedite the POD, all faculty and staff members who plan to receive their flu shots at this event are asked to print the Influenza Vaccine Screening and Consent form and Vaccine Information Sheet, found at the following link and bring the filled out form to the Fitness Center: https://www.ithaca.edu/sacl/healthcenter/docs/InfluenzaScreening.pdf
A POD, or Point of Dispensing, is a location where life saving medications, supplies or equipment can be dispensed or distributed. Should there be a bio-terrorist attack or other disease outbreak, the Centers for Disease Control (CDC) recommends that communities dispense antibiotics and/or vaccines to all members of the community in a 48-hour time period.
Following is a schedule based on faculty and staff locations. All supervisors are encouraged to support faculty and staff participation.
If at all possible, come to the Fitness Center during your assigned time for the flu vaccine. If the location time does not work for your schedule and you wish to participate, please come any time during 7:00 a.m. – 12:00 p.m.
AGAIN, the Influenza Vaccine Screening and Consent form and Vaccine Information Sheet are intended to be completed on the same day that you are receiving the flu shot, so please fill it out in the morning and bring it with you to the POD event. This will help the entire event to run smoothly. We greatly appreciate your cooperation!
