Please note: Risk Management can only accept the most current driver information form (revised January 2016). Should your department or area have older copies of the driver information form, please dispose of them.
Also on this site you will find the Motor Vehicle Accident (MVA) Procedures document, which every driver should be familiar with if involved in an accident.
There is also useful information regarding domestic and international vehicle rentals from an outside agency.
We request that an employee submit a driver information form only when they have a need to drive; since there is a cost involved in obtaining an individual's Motor Vehicle Record.
