Starting on May 1, 2018 HSA members can view their 5498-SA tax form on the PayFlex member website. If you elected to get a paper copy, PayFlex will mail it to you. You don’t need it to file your taxes. But you should keep this form with your tax records. Form 5498-SA includes pretax contributions from your paycheck, any contributions you made on their own (post-tax), and any employer contributions. These are contributions posted to your HSA for 2017. PayFlex also send a copy of the form to the IRS.
