Things to note:

Online requests are for payroll deduction only. If you would like to purchase a meal plan with a credit or debit card, please click here to download our form.
We cannot accept photos of a downloaded form. Please choose a .PDF or Word copy, fill it out, save and attach it to an email. If you're filling the form by hand, please scan it and e-mail it to u.
While most forms are processed same-day. We ask that you allow 1 business day for meals to be added to your account. If you submit a form after 4:00PM or on a weekend, your request will be processed the next business day.
