2,600 IC students were randomly selected to complete the on-line National College Health Assessment Survey.

If you received an e-mail with the heading "Reminder - Health Promotion Survey" from NCHA-WEB, we encourage you to complete the survey which takes about 15 minutes. You might win the grand prize of $100, or 2nd prize - a $50 gift certificate to Wegmans; or you might be one of 50 students to win Smoothie Coupons for the Grand Central Cafe.

Questions include: demographic information, preventive health, academic impacts, sexual behavior, nutrition, excercise and depression.

Please read the introductory letter carefully - it will explain the measures that have been taken to assure your privacy in answering the survey.
