When providing new staff or student drivers with the driver information forms or renewing your own form, please make sure you use the current form located at: https://www.ithaca.edu/riskmanagement/forms/mvr_welcome_page.htm Previous versions of the form will no longer be accepted since there have been several changes. Once submitted, a minimum of 14 business days is required to process the form.
Submitting your driving information via this form is required to obtain the privilege to drive vehicles that are owned, leased, or rented by the College (including those from a rental agency). The final decision of whether or not a person can drive such vehicle(s) rests solely with the College's insurance company’s report, based on a driver's record. By signing the driver information form you authorize the release of your driving record to IC's Office of Risk Management; Chubb Insurance Co. and Arthur J. Gallagher and Co. of New York, Inc. (Ithaca College's insurance carrier and broker, respectively); and driving status to your direct Ithaca College supervisor. Please contact the Office of Risk Management with any questions.
