Did you receive an e-mail recently regarding an IC Health Survey?

Randomly selected students were chosen to take the National College Health Assessment online health survey this month, sponsored by the Health Promotion Program. The information regarding students' fitness, nutrition, sexual behavior, alcohol and drug use, and mental health is very valuable to us -- so, we're offering a number of prizes for completion. How about a $100 gift certificate to your favorite local store? 

Here's a list of the prizes:

• Grand prize: $100 gift certificate to a local store of your choice
• Second prize: $50 gift certificate to Wegmans
• 50 chances to win a smoothie at Grand Central Cafe

Please take 15 minutes to complete the survey!  If you were selected to participate, but haven't completed it yet, you'll receive a reminder e-mail sometime this week.  The end date for completion is April 27.
