The Wellness Clinic is now accepting new membership applications. If you would like to join, just go to our website (https://www.ithaca.edu/hshp/clinics/wellness/) and click on "Join The Clinic." On the next page you will see the link to the Health History Form. Just fill it out and it will be e-mailed directly to us. Someone will get back in touch with you within a week. |
