Pursuant to 29 CFR Part 1910.1020 of the federal Occupational Safety and Health Administration (OSHA) regulations, you or your designated representative are entitled to examine and copy relevant information pertaining to your exposure and medical records.

An exposure record is a document containing information on workplace environmental monitoring, biological monitoring (e.g., level of a chemical in blood), material safety data sheets (MSDS), chemical inventories, and any other record that identifies the type, as well as where and when a toxic substance or harmful physical agent was used. A medical record can be a medical and employment questionnaire; results of medical examinations; laboratory tests; written medical opinion, recommendation, diagnosis, or notes; first aid record; a description of treatment; and employee medical complaints.

Records are located and maintained in several college departments:  Health Center, Human Resources, & Environmental Health & Safety (EH&S), however, all written requests for records should be made to EH&S.  “Records Request Forms” are located in the Office of Human Resources located in the Peggy Ryan Williams Center or at EH&S. A printable “Records Request Form” or a copy of this notice can be downloaded from the EH&S website at https://www.ithaca.edu/sacl/safety/docs/ehsworkplace/recordsrequestform.pdf
