Digital Instruction and Information Services (DIIS) is pleased to announce that most of our account request forms are now online! No more printing, filling out by hand, and schlepping around for required signatures!

The following forms can now be submitted electronically:

  • Active Directory Generic Account
  • Department Email and Calendar Account
  • Majordomo Mailing List Account Request
  • Student Organization Email Account
  • Voice Mail Account

The new forms can be accessed through IC Workflow, which is available in the left-hand menu of myHome. Sign into IC Workflow, select the appropriate form, fill it out online, and submit! Your request will be electronically routed to the designated authorized approver (selected during completion of the form), and once approved, submitted into the DIIS Service Desk ticketing system for processing.

The new process will eliminate problems with incomplete, difficult-to-read, or misplaced paper forms, allowing DIIS to process your request more efficiently. We hope you find the new process to be convenient!
