Keynote Speaker, Tom Caprio, MD, MPH, MS, CMD, HMDC, FACP, AGSF, FAAHPM will open our day together discussing how the demographic shift in our communities impacts multiple stakeholders.

 “With the demographic shift of an aging population and the progression of chronic diseases, an imperative for palliative care has evolved with multiple stakeholders which range from patients, families, health systems, government, and the community of context for those in need.  In an effort to provide the best quality of care, reduce suffering, and improve quality of life there is an opportunity to use the lessons from geriatric medicine and palliative care to create a new path forward for inclusion of those most vulnerable in our society supported by an engaged and active community of champions in improving the “status quo” of a medical model in health system focused on disease cure and high technology interventions.  The opportunities for creative thinking, including an emphasis on advance care planning and “primary” palliative care are key stepping-stones for sustainable change on a large scale.”
