Ithaca College

Ithaca College recently acquired Formstack, a web form application that will directly replace JotForms (within WPM) and Cognito Forms. Formstack offers the self-service ability to create dynamic forms for registrations, surveys, and workflows, with the ability to collect online payments and HIPAA-related information.

The Business Productivity team has been reaching out to existing form owners of JotForms and Cognito Forms as part of a phased winding down of those services by February 2021.

• If you already have access to build forms in Formstack, or if you need to complete workflows, you can log in through apps.ithaca.edu, or https://ithaca.edu/formstack

We have additional opportunities with Formstack that include the use of Formstack Documents, a seamless document creator, and Formstack Sign, an electronic signature service. We will be working with departments and our vendor to identify how these features can be leveraged by individual departments.
